Are you new (new to OASIS)?
All new PAC/MAC must submit an NC Substitute W-9 Form to IDS. The NC Sub W-9 must be sent to IDS so we can update our records and then send the information to the Office of the State Controller (“OSC”) for processing. And the information you provide on the NC Sub W-9 must match IRS records.
The Sub W-9 should be mailed to Indigent Defense Services,

Or fax securely to 919-890-1922.
If your Tax Identification Number (“TIN”) changes, you must complete a new NC Sub W-9.
Has your information changed?
***If any of the following information changes, you must complete an NC Supplier Change Form.
- Business / Law Firm Name
- Business Address
- Personal Name (marriage, divorce)
