Attorney Fee Application Forms

Note: IDS requires attorneys to submit their fee applications within one year of final disposition. In advance of the deadline an attorney may seek a thirty-day extension, if good cause is shown. Under compelling circumstances an attorney may seek a waiver of the deadline.
