Are you new (new to OASIS)?
All new PAC/MAC must submit an NC Substitute W-9 Form to IDS. The Substitute W-9 must be sent to IDS so we can update our records and then send the information to the Office of State Controller (“OSC”) for processing.
The Sub W-9 should be mailed to Indigent Defense Services, c/o Shannon Simmons, PO Box 2448, Raleigh, NC 27602.
Or fax securely to 919-890-1922.
The information you provide on the Sub W-9 must match IRS records:
- Tax identification number (hereby “TIN”); or
- Business or law firm name; or
- Business address; or
- Personal name (marriage, divorce)
Has your information changed?
***If any of the above information changes, you must complete an NC Supplier Change Form.
The NC Supplier Change Form should be mailed to Indigent Defense Services, c/o Shannon Simmons, PO Box 2448, Raleigh, NC 27602.
